All selected Fellows and Alternates must upload a clear, complete copy of the photo page of their passports. This scan is used by IREX to prepare your J-1 visa. Before uploading, please make sure all numbers and letters are included and legible, that it is signed, and that the photo is clear.  Please complete the following form and upload a copy of the photo page of your passport by August 9, 2023.

Passport requirements:

 

• Passport must be from one of the 37 Fellowship-eligible countries in the Western Hemisphere.
   
• Must be a personal passport (not a diplomatic passport, state official passport, or passport card.)
   
• Must be valid until July 31, 2024. If your passport expires before July 31, 2024, please begin the process of renewing your passport immediately.

This form is to provide important medical information to health care providers in the event of an emergency. Please be open and honest in all of your answers so that IREX and our partners may be prepared to assist you. The information on this form will not in any way impact program selection or participation.

Fellow Headshot Guidance

Fellow headshots are going to be featured on the IREX website and shared with Embassies and Consulates and online. When uploading your headshot, please make sure to read the guidelines closely before submitting your headshot. 

If you feel comfortable doing so, please also submit your social media handles in the appropriate space provided.

Please complete this survey with all personal details as it appears on your passport no later than August 9, 2023. Our team will use these details to make all necessary travel, visa and lodging arrangements on your behalf. 

Our team will also use the demographic and business sector information you provide in this survey that will help our team prepare our pre-arrival curriculum and conduct outreach to U.S. Organizations for the in-person portion of the program accordingly.
